Rajiv, NTA implies that mentioning name and date on the photo to be uploaded is important. But not mentioning name and date on the photo doesn't call for a rejection of the application form. If NTA thinks that the uploaded photo doesn't fall in line with the guidelines, you will be notified at your Registered Email id and Mobile Number and then you can upload another image. But if you do not receive any notification, consider the  uploaded image to be good enough.
