Hope you are in good health , so sorry to hear about this but according to my best of knowledge NEET application form is very much important for counselling purpose , in counselling they will always try to match the documents with correct Name, address , 12th marks , so  I would say please immediately contact the NTA  helpline number and try to solve this matter because it will surely become a difficulty in counselling time.
