Medicine is your passion & fashion designing is your hobby. Sharing a personal experience : My friend was in PCB section gave neet was very much into medicine but entered NIFT & was never satisfied. 

your hobby can never satisfy you but your passion will satisfy you & feed you 

ultimately the choice is yours

Medical and Fashion designing are two very separate fields. You will eventually have to choose between the two. I would suggest you to go with your passion. Hobbies change, passion doesnt. Also you can side by side work as an amateur fashion designer and learn from youtube. But again. It all depends on you.
Just follow your heart.
