ECFMG states that effective 2023, Doctors applying for EFMCG certificatiom should have graduated from a College that is appropriately accredited. If you are planning to study MBBS in India, all colleges would be recognised by MCI. By 2021, MCI would have been replaced by NMC and all Colleges participating in All India Counselling are duly accredited. So even if you join MBBS in India in 2021, you would still be eligible to take USMLE once you complete your MBBS. If you start your course in 2021, as it is a 5 and 1/2 year course, you will complete it by 2026 approximately. So you cannot take USMLE in 2023 but can take it after you complete the course.
