No it's not mandatory to upload the candidate picture with name and date. But it must be a recent photograph.

No, it's not mendatory (if you have uploaded a photo-identity proof) but for security you should upload the photograph with name and date of picture because the recent photograph is required while filling the NEET 2019 Application Form. If you want to change  you can correct your photograph or other information in the NEET 2019 application form from 14th January 2019 to 31st January 2019 on the official website NEET.
