in neet form in column for 12th class % I have fill my % which is written on marksheet (which include PCB , english, physical education) or I have to fill only PCB and english % collectively ?

No you don't have to count your mark from English and physical education. You'll just have to count physics, chemistry and biology marks for counting your qualification percentage. But, you should have passed in the other two subjects to attend NEET exam , though their marks doesn't matter.

Your PCB percentage needs to be more than 50% in case you belong to general category. If you're from OBC, SC and ST category your marks need to be more than 40% . For PWD candidates they need to score more than 45% here.
