In neet form what is meaning of state of eligibility criteria

In the field asking for 'State of Eligibility Criteria', you should fill  the state where your permanenet address is located or the state where you have full domicile eligibility criteria for 85% state quota seats.

Can we fill more than one state in neet form under 'state of eligibility ' column?

I have done my class 12 from Uttar Pradesh but I have domicile of uttarakhand and in admit it is showing state of eligibility is uttarpradesh can u explain
