There is no predefined format in which the date and name should be mentioned on the uploaded photo. So you are absolutely fine with what you entered on the uploaded image. You need not upload any other image. You can only correct uploaded images if you receive a notification asking you to do so. If you haven't received any such notification, you are good to go.
