See ,truly speaking you do not have an excellent rank.If you belong to general category and you want to take admision under All India Quota seats then you won't get the seat for Dermatology. The courses like Radiology, Dermetology, Neurology are especially given to toppers. if you get admission through state quota or under some reserved category, you might get dermatology but sorry to inform you that chanes are very low if you want to take admission in a good reputed Government College.
