I hope you are doing fine, You can get an affidavit done from court stating that both the names belongs to your father and you can provide the same at the time of document verification if necessary. You need to fill your details such as your name, father's name, mother's name and date of birth as per the details in class 12th certificate. If you do so you will not face any problem.
