Actually, codes depend on your current qualification like 12th appearing or passed out or doing bsc or passed bsc or the board from which you have done 12th etc and not whether you are appearing for the 1st or 2nd time etc.

I am assuming that you are class 12th appearing 2021 student and that is why you it's your 1st attempt.

If that is the case then,

You need to choose code 1 :-

This is because code   1 is for 12th board 2021 appearing students or have already appeared for 12th board 2021 examination but results are awaited i. e results are not declared yet.

Here's a short brief of Neet codes :-

--------) Code 1 :- code 1 is for class 12th board 2021  examination appearing students with physics, chemistry, biology and English or have have already appeared but results are awaited i. e have not been declared.

--------) Code 2 :- code 2 is for all those who have passed their 10+2/higher /senior secondary/ Indian School Certificate Examination with physics, chemistry, biology and English

--------) Code 3 :- code 3 says it is for those passed their intermediate / pre- degree examination  with physics, chemistry, biology and English ,your physics, chemistry and biology should also contain practical exam.

--------) Code 4 :- pre - medical degree with Physics, chemistry and biology.

--------) Code 5 :- code 5 is for all those who are doing BSc and has all the three of Physics, chemistry and biology in their graduation.

--------) Code 6 :- code 6 is for all those students who have passed  BSc  with at least any two of following subjects :-

--------) Code 7 :- code 7 is for those who have passed their examination from foreign board which is equivalent of Indian 10+2 board.
