I am extremely sorry to tell you dear but with just 10th qualification you are not eligible for neet examination, there's no such form . To be eligible for Neet examination, it is mandatory to be either appearing for class 12th examination or should have passed it with a minimum of 50% aggregate marks taken together in Physics, Chemistry and Biology/Bio-technology in your class 12th board examination from a recognized board if you belong to general category ; while it's only 40% aggregate in pcb for obc, sc and st categories , 45 % aggregate in pcb for pwd category. Also to be eligible for Neet 2020 you need to be at least 17 years old or more by 31st December , 2020 .
