See performa is nothing but a pdf of admit card which you have to download and take print out of it. They wanted to say that you have to past a postcard sized photograph on your admit card which should be handed over at the examination centre. Along with that admit card you will find an undertaking which is nothing but a self declaration form which signifies that the candidate is not suffering from any of the symptoms of COVID-19 and this form should be signed by the candidate in front of an invigilator present at examination center. If there will be no admit card or self declaration form you will not be allowed to give examination. Along with the admit card you should also carry at least two same passport size photographs as pasted on your admit card. These two photographs will be taken from you at the examination center for attendance.
