Should I have to self attest the photocopies of comed k document and do I have to mention my name and other details on demand draft(origina)

Yes, You need to self attest the comedk photocopy documents , by self attest  it means that you need to sign the document and mention "self attested" . Yes, you have to fill in your personal details in the demand draft.
