While filling the form one needs to fill her / her class 12 Roll number. Thus, you need fill the Roll number which you have on your class 12th certificate and not the Roll number of single additional subject . During document verification you will have to carry both certificates .

• General-EWS/ OBC-NCL :- Rs 1500
