UDID Card will have all the relevant details related with the person and her disability. This information can be easily read with a card reader device.

You can use the UDID card instead of the main certificate as it carries equal importance to the certificate. But if you have the CMO certificate, i will suggest you to upload it. Incase the size is more, there are online tools to crop or cut down the size for free
