The details which are in class 10th certificate are considered while taking admission. So you need to fill all the details as per class 12th certificate in the application form. Since the name in class 12th certificate and class 10th certificate is different you can get an affidavit stating that both the names belong to your mother and you can provide the affidavit at the time of admission.
