1. UPSEE 2020 rank card
2. UPSEE 2020 admit card
3. 10th and 12th standard certificate and mark sheet
4. Domicile certificate
5. Parent's domicile certificate (if the candidate has passed qualifying exam outside Uttar Pradesh)
6. Rural weightage certificate (if applicable)
7. Character certificate
8. Category certificate (if applicable)
9. Subcategory certificate for freedom fighters/armed forces/physically handicapped (if applicable)
10. Certificate of medical fitness/undertaking of medical fitness
11. Income certificate for economically weak candidates
12. GDDA category certificate (if applicable)
