An Undertaking that is also known to be as an the self declaration form is a  document created by either a court or the police that places the person being charged with an offence under certain conditions and in JEE Mains also it means the same.  The JEE Mains admit card released has a self declaration (undertaking) form for providing details of travel history in the last 14 days and symptoms of covid-19, if any, which every candidate has to undersign. NTA has also prescribed a dress code where shoes/footwear with thick soles and garments with large buttons are NOT permitted. So these are the points that are basically mentioned in the JEE Mains declaration or undertaking form.
