• High School & Intermediate Passing Certificate
• Identity Proof
• Birth Certificate.
• Category Certificate (If Applicable)
• Passport size photograph (Six)
• NEET Admit Card
• NEET Rank Card
• Provisional allotment letter

To have a personalised report chances of getting a college you can go through college predictor. It will provide you complete list of colleges for all india quota and state quota separately on which you can get admission on the basis of your category, domicile state etc.

• 12th Certificate
• SSLC or Transfer Certificate
• Domicile Certificate of Gujarat State
• NEET Mark Sheet
• Document showing date of birth, place of birth : SSLC/Passport/TC/Birth Certificate
• Caste Certificate for SC/ST/SEBC candidates
• Non-creamy Layer Certificate for SEBC candidates
• EWS certificate issued by competent authority of Gujarat State.
• Certificate of disability, if you belong to this category
• Documentary evidence in case of NRI
• Local candidate certificate from the Dean of NHLMMC, Ahmedabad or SMIMER, Surat based on the locality.
