The following documents are required for the NEET application form:

1. Candidates must have valid Mobile Number

2. Candidates must have validEmail Address

3. Candidates must have Class 10 details and Class 12 details

4. A photo-identity proof - Aadhaar Card number, Passport Number, ration card number, bank account, any other valid Govt. identity number

5. Scanned images of passport sized photograph, and signature and other documents (if applicable)

6. Credit/Debit card or Net Banking details (for online payment)
