When applying for JEE Advanced , the candidate needs to upload scanned copies of: Birth Certificate or Class 10 certificate as birth proof Class XII (or equivalent) examination Category PwD (if applicable) Scribe Request Letter (if applicable) PIO Card / OCI Certificate (if applicable) DS certificate (if applicable) Gazette notification showing name change ( if the name is not same as in Class X certificate) Documents to be submitted by reserved category candidates: OBC-NCL: OBC-NCL certificate (form EC2) issued by the Government of India SC/ST: Caste (for SC) or tribe (for ST) certificate (form-SC/ST) PwD: Physical Disability certificate (FORM-PwD II/III/IV whichever is applicable) issued by the notified medical authority. Candidates who need scribe service: Letter issued by Chairman of respective zonal coordinating IIT (FORM-SCRIBE). DS category: Certificate (FORM-DS) issued by Directorate of Resettlement and Rehabilitation, New Delhi under the Ministry of Defence or the Ministry of Home Affairs, Govt. of India. PIO/OCI candidates: PIO card or OCI certificate, whichever is applicable. GEN EWS Certificate if applying under the EWS quota. In addition, a photograph and signature as specified will also have to be uploaded along with the application.
