Due to Covid-19 pandemic, the students who are appearing for JEE or any other exam need to submit self declaration form, stating that they are not tested positive for coronavirus and they also don't have any symptoms like cough, fever  and breathing related issues.  The self declaration form must be filled before the exam with your photograph and it should be signed in the presence of invigilator at examination centre.

The self declaration form is attached with admit card, so when you will download admit card you  will also get self declaration form.

After releasing Jee mains admit Card in September NTA asks Students to download self declaration form and fill the necessary detaila like past travel history,any covid symptoms,etc.
So take it seriously and fill all the mandatory detail and you have to reach center on time with admit card,self declaration form ,photo, identify etc. On time.
