You may also fill provisional certificate (in the PDF link) and then give the original one by 5th June 2019.

Hello,

The format for OBC-NCL Certificate is given in the official website of JEE Advanced 2019. You can view the format as well as download it from https://jeeadv.ac.in/downloads.html or https://jeeadv.ac.in/documents/certificates/OBC-NCL.pdf

The last date for applying is 9th May 2019. So please make it fast.
