Hello there

It is just a medical certificate which is required for every student during the counselling of NEET.

For it you can get the performa from the internet and this varies with college to college and you can search out of your college in which you are taking admission.

You have to sign the certificate from a MCI recognised doctor and the doctor will have to put his stamp on your certificate provided their stamp will be containing the registration number of the doctor who is stamping on the certificate.
