so according to code 5 ,candidates who studying in the first year of the three years’ degree course of a recognized University with PCB (that includes practical tests in the given subjects) provided the examination is a University Examination. The candidate also passed their earlier qualifying examination with Physics , Chemistry , and Biology/Biotechnology with English at a level not less than a core course.

Asiri if you have all of physics, chemistry and biology in your bsc then you need to select the code 05 but if you don't have physics, chemistry and biology in your bsc then you need to choose the code 02 on the basis of 10+2 qualification .
But yes dear you are eligible for neet only if you have aggregate of 50% in physics, chemistry and biology in your 12th examination if you belong to general category but only 40% aggregate is required if you have belong to obc, sc or st category.
