Sports quota is only applicable for the candidates who have national or any intermediate level certificate in any kind of sport. The only way to get sports quota certificate is to participate in state or national level sports. The wining certificate is your sports quota certificate. You need to submit those certificates during NEET counselling. That is the only possible way to enter sports quota.
