See for NEET 2021 application no special document will be needed, however I am enlisting down the list of document which you will need for uploading at the time of application :-

• Candidate’s latest Passport size Photograph
• Latest Post Card size photograph (4”X6”) of candidate
• Candidate’s Signature
• Left thumb impression of the candidate
• Class X passing certificate

Note that as the application process is completely online so you should have all above documents and photographs in soft copy or in scanned image form with specific size and format as prescribed by National Testing Agency, you can check out all those details from the link given below :-

Also till now National Testing Agency which is the conducting authority for NEET, has not released any notification for NEET 2021 and until any notifications get released by we cannot help you with exact dates.
