It's preferable to start coaching in 3rd year Mbbs.
Then there is less pressure during internship and one is more prepared for entrance.
3rd year also gives the time for doing self study, going to coaching classes and attending college lectures.
Some students also prefer to do coaching during internship or take a drop and do it later.
It totally depends on you as when you want to time it.

You should start taking coaching from 3rd year itself because clearing md is not an easy task and it requires lot of preparation and hard work . So according to me a student willing to pursue md should start preparing early in mbbs days itself and at any case from 3rd year itself
