Your name in the class 10th certificate and the application form should be the same. In most of the application the candidate is asked asked to fill the personal details such as candidate's name, father's name, mother's name and date of birth as per the class 10th certificate. So the candidate has to fill the application form as per the information in the class 10th certificate unless stated the otherwise. You might face problem at the time of document verification as the name in the application form and the class 10th certificate is different. You have not mentioned the exam you have applied. But if the applications are still open and if multiple applications are allowed then fill the application one more time and pay the fee once exam. You need to appear for the exam once and consider the second application if you would fill another application. If the last date is over or the multiple applications are not allowed get an affidavit stating that both the names belong to you. You can provide this affidavit at the time of document verification and you won't face any problem.
