You havent mentioned which State you belong to. But the Medical Board constituted by the Competent authority will have to examine and assess the percentage of disability of disabled candidates. The official notification for most States states that they follow the guidelines set by MCI. It is likely that the disability assessment certificate should be procured from from any of the Centres mentioned in their official website at mcc.nic.in

Hello dear student, 

If you are getting admission under PWD quota be it anywhere state or all india you have to present the disability certificate to prove it. I hope you understand this because if you want a seat and you are getting a seat with that person with diabetes quota then certificate should be presented by you at time of document verification.
