According to the previous year cut off, the cut off score for nalanda medical college for sc category for All India quota was 436 which is less than 454 so you may have a chance to get a medical seat in that college but for patna medical college the cut off was 487 which is way more than 454 hence you have low to no chance to get a medical seat in that college.

Please keep in mind that each year cut off variates a lot due to change in pattern of the exam, no. of exam takers, level of difficulty of the exam hence this year too the cut off may change (may go up or come down).
