Hi Dhanush,
NEET documentation requires your full fleged marksheet consisting of all subject marks and date of birth. Therefore,it would be better if you upload your school's certificate if it contains all your information so as to avoid the risk of getting your application rejected. Do follow the guidelines properly.
You may also ask the officials for further clarity on this.
