Hey Khushi !
Yes, there will be different forms required to fill up for the 15% All India Quota as well as 85% State Quota.
The 15% quota is filled up by mcc which would be conducted centrally whereas the 85% of the same would be done only for the State Quota which also corresponds to the fact that you must own a domicile for being eligible to own a seat through State Quota.
