Yes, you have to register again but now you have to do some less effort than before,

Just go JEE Main official website

Click on apply for JEE Mains 2019

and register under JEE (Main) January 2019 Registered Candidates

Sign in with your JEE Main January credential and the details which you filled in January mains application will be already filled just you have to pay the examination fees and you will be successfully registered for JEE Mains 2019 April exam.

Hello aspirant, even though the exams are same, but they are different in sessions, so you need to pay once again for the JEE mains april exam even though you have paid for the January session exam. When registering for the april exam, sign in with the login credentials you have used for january exam and your details will be automatically filled.

Yes, If you apply for JEE Main (April Attempt), you will have to pay the application fee (which you can find at the following link, https://engineering.careers360.com/articles/jee-main-application-form ) irrespective of whether you have applied for JEE Main Jan attempt or not.
