2.mothers name 

3.fathers name

4. date of birth 

5. category 

and submit. you will be able to get your application number. hope you save the application number this time.

I will tell you how to get it back step by step. First thing you have to do Is open NEET Official website cbseneet.nic.in and then click on fill application form or login for admit card or login for image correction or challenge answer keys and then on the right side you can see I cant access my account. Now for recovering your application number you will need to fill in below required information but the same mentioned during online application for example Candidates Name :Mothers Name :Fathers Name :Date of Birth : (Day Month Year  Format)Security Pin (case sensitive) : (Auto Generated Pin, which is displayed bottom of the form) etc. And once you have filled the required information  Click on Submit Button to get back your application Number or Password. And then In case you are not able to retrieve your Application Number or Roll Number using the recovery option mentioned above.
Write email to net@cbse.gov.in , I will give you a sample email as well requesting for Lost application or roll Number.

Email subject:Reg.LostRoll Number of NEET Exam Conducted on DD-MMM-YYYY
Email body: Dear Sir/Madam,
I have appeared in NEET Exam conducted on date (DD-MMM-YYYY), and I lost My application Number. I would like to request you to provide my Roll Number so that I can check my Results or appear in NEET.

Dear Sir/Ma'm, while doing the registration, you must have got a message on your registered mobile number as well as e-mail id.
