Hii!
It will be clearly mentioned in the NEET UG application form that you have to fill in the details such as your name, your father's name, mother's name, DOB as mentioned in your class 10th marksheet. Id proof can be anything, but the details should match with that on class 10th marksheet. Also, read the guidelines carefully before filling the form.
