With 428734 rank in NEET 2025 under EWS category, chances for MBBS in Gujarat through government quota are not possible. The NEET cutoff for EWS in government colleges stays much lower.

In private colleges under government quota also, chances are very low as cutoffs usually close around 2 lakh rank.

Even for management quota seats in private colleges of Gujarat , your rank is quite high. So chances are very less.

You can try for MBBS in other states where private colleges take higher ranks, or you can consider BDS, BAMS or other medical-related courses.
