As you have mentioned that your neet rank is 38816 and your score is 518 which is not that good to secure a seat under All India Quota.
Now, looking over previous year's cutoff :
You haven't mentioned your category. But considering you under general category (UR), the AIQ neet cut off for 2018 ranges from 590 to 552 marks.Also 533 marks is the cut off through AIQ for government dental college.So there is absolutely no chance of getting a government college through AIQ.
As far as state quota is concerned,The cut off ranges from 550 to 518 marks in neet 2018..
After analysing last year's trends, I can say that you have really very less chance of getting a medical seat either through state quota or All India quota. As cutoff in both cases are very high.
