Dear Satyam, if you haven't submitted the form yet, please donot submit it. Login with your credentials, click on the 'Review' button and go to 'Edit' option. Edit the 'State of Eligibility' and fill in the form. Please remember to fill in the details like State of Eligibility, PWD status, Category, Gender and Nationality correct because you cannot correct them even in the correction window.
