Dear Candidate,
To appear for NEET you must have passed 10+2 or equivalent with physics,chemistry,biology/biotechnology and english as your subject in 10+2.Class 12 or equivalent appearing student are also eligibile to appear for NEET.Candidates must complete their 17 years of age in December.
The class 12 percent required for NEET exam is different for different category. General category candidate must score 50% agreegate marks in 12th to appear for NEET while OBC/SC/ST candidate must score 40% agreegate marks in 12th and PWD candidate must score 45% aggregate marks.

You need to pass in all subjects such as physics, chemistry, matha and biology with 50% score for general candidate and 40% marks for SC/ST/OBC and 45% for PWD students. However, along with this you need to clear the minimum cut-off marks of the neet examination and the cut-off changes every year. It depends upon a lot of factors such as the exam pattern, the number of students appeared, the number of students qualified, whether you belong to any category or not, the highest score etc.
