The DU admission form for merit based admission and entrance based is same, so you don't need to fill two different forms.
Candidate seeking for admission need to fill only a single form for admission in both entrance based and merit based courses.

No, you do not need to make any changes in your form if you have updated your marks after result declaration. Now, for merit based admission you just need to check and wait for the cut offs and take admission in case you qualify the cut off of any college. also, it is to be noted that Delhi university take admission in  all merit based as well as entrance based courses through the same online application.
