Hello Sakshi,
Anything could not be said about what colleges you can get on the basis of your score in NEET, you should know that it would be too early to predict anything. The cut off trends tend to change every year and same can be expected for this year as well. However, for your given score, you have a feeble chance to get into a good college. If you want a decent level college you should have achieved a score of 500+ as an UR candidate. But you can try the college predictor for further information about colleges:

I'm sorry to say but with 491 it's a tough call for you to get a government medical college in Madhya Pradesh. This state has a lot of seat but due to a lot of applicants the cut offs go really high. You need to score atleast 520+ and this year the cut offs are expected to go higher than last year. Still don't lose hope, you may not know what can happen. Do apply for the counseling.
