Dear Aspirant,
No you needn't fill form for Jaipur campus separately. All the campuses are included in the university. The score card of the common entrance exam of Manipal University can be shown to any of the campuses ( Manipal, Sikkim, Jaipur). There is only one entrance exam. I am providing you a link which you should check out. It contains all the relevant information on MET.
