You don't need NEET application form , If you have confirmation page.

You need only these documents on the time of counselling

• NEET 2018 Admit Card
• Confirmation page
• NEET 2018 Rank Letter
• Date of birth proof
• Class 10th pass certificate
• Class 12th pass certificate
• Class 12th marks sheet
• Eight passport size photographs. These should be same as the one used in application.
• Provisional allotment letter
• Proof of identity
• Reserved candidates should carry proof of the same, issued by competent authority
