I am sorry to say but a score of 286 is too less to ensure you a seat in any of the Govt. Medical Colleges in the state of Kerela under State Quota even through SC Category.
If we look at the statistics of 2018, the last candidate to get in through Kerela State Quota under SC Category had a rank of 89,309.
Although you haven't mentioned about your rank, but after this year's Rank Inflation, it would seem quite clear that your rank is well beyond 1.5 lakhs which is quite far to make you elligible.
You can try to look out for BAMS or BHMS though through the rank achieved.
