Yeah.. you may face lot of problem. And in extreme case your candidature might get rejected.
According to neet ug official notification , the pic must be in white background with the name of candidate and date of taking pic , which your pic doesn't follow the same. So i guess your application form might get rejected.
However you can talk to neet ug officials once, may be they could help in this regard.
