Now AIIMs admission is done on the basis of the All India Rank you get in NEET UG. Since you have 70% physically handicap certificate you need to get a rank below 3,786 for AIIMs Delhi (highest) and around 30000 AIR for AIIMS, Guntur.

To get admission in AIIMs you need to get a minimum of 550 even if you have a phd certificate, however the marks may increase depending on the number of students and the difficulty level of the paper.
