If you are going to appear for exam of class 12th in 2019, then YES you are eligible to fill the form this year. But if you are going in 12th class in 2019, i.e currrently you are in class 11th, then NO, you arent eligible for it this year.   

No, You will be eligible to appear for NEET next year Onwards, Becuase You must be in class12th Minimum to appear for NEET, I Don't know how You had different subjects Physics, Chemistry and Biology in class12th Where in Most of the courses it is class hs Science as a Subject.
