November 30th in 2018 is last date to fill and submit application forms for NEET UG 2019 attempt. 

Students have to choose exam centres and slots and pay fees online.

NEET 2019 application forms will be released by NTA on November 1, 2018 in online mode.
Last date to fill the online application form will be November 30, 2018.
